Without a signed copy of this release form we must decline to proceed with your repair.

We recommend that you double box leaving at least two to three inches around each box using bubble-pack or popcorn. Each item should be wrapped separately with bubble wrap separating each item. Or use UPS or Fed-EX for packing and shipping and insurance is recommended.

Be sure to enclose your name, address, phone number and email address on this release form.

There are certain unavoidable risks in crystal and glass repair. You must understand that the piece to be repaired is already damaged and on rare occasions, during the repair process, a piece can shatter due to hidden cracks, faults, internal stress or by accident. We can assure you that utmost care will be given in the repairing process. All liability for breakage and results remain that of the customer. Shipping damage is the responsibility of the shipper.
